I certify that the statements made in this application, on my resume and any other attachments, and any other information that I provide to Minted Provisions Inc. are true and correct to the best of my knowledge and that I am not using / will not bring any presentations, templates, emails or other information from my prior contracts that could be in violation of any confidentiality agreements or trade secret laws. I understand that any misinformation or falsification of information on this application or any other document will be cause for termination of my contract.

I acknowledge that all contract offers are contingent upon the satisfactory completion of background checks. I agree to provide any necessary consent forms required for Minted Provisions Inc. to conduct its lawful pre-engagement checks. By submitting this form, I authorize all relevant parties, including but not limited to previous clients, companies, corporations, and educational institutions, to supply Minted Provisions Inc. with any pertinent information regarding my background, and I release them from any liability arising from their cooperation.

Should my contract with Minted Provisions Inc. terminate, I understand that Minted Provisions Inc. may disclose my complete record in response to any legitimate request, and I release Minted Provisions Inc. and its representatives from any liability in connection with such disclosure.

If contracted, I agree to comply with all Company rules and regulations. I understand that my contract with Minted Provisions Inc. may be terminated at any time by either party, with or without cause.
